Mail My Flatiron Meal Plan Card

Please complete the form below to have your Flatiron Meal Plan photo ID card mailed to you. Cards will be shipped by USPS and should arrive in 3-5 business days. If you would like to receive your card right away, please stop by our office on the Hill at 1163 13th Street, Unit A (right next door to The Sink restaurant) and we can make your card on the spot. Office hours are M-F, 10AM-4PM.
